The nurses at TCMS are Zoe Wilmot, RN, and Kelsey Hadzima, RN (currently on maternity leave). Our office provides health services to students and serves as an advocate, liaison, and consultant on health-related issues for students, families, and school personnel. We also help support an optimum physical, mental, emotional, and social climate for all students and staff.

2026–2027 New York State Health Requirements

Students Entering 6th Grade: Tdap Vaccine Required

All students entering 6th grade must receive one dose of the Tdap vaccine between their 10th and 11th birthdays. This requirement is different from the DTaP vaccine received during early childhood.

  • A Tdap dose given before age 10 does not meet the New York State requirement.
  • If your child has not yet received the Tdap vaccine, please schedule an appointment with their healthcare provider and notify the TCMS Health Office of the appointment date.
  • If your child has already received the vaccine, please send a copy of their immunization record to the TCMS Health Office.

Students Entering 7th Grade: Meningococcal Vaccine Required

All students entering 7th grade must receive the meningococcal vaccine.

  • A dose given before age 10 does not meet the New York State requirement.
  • If your child has not yet received the vaccine, please let the TCMS Health Office know the date of their appointment.
  • If your child has already received the vaccine, please submit a copy of their immunization record to the TCMS Health Office.

Students Entering 7th Grade: Physical Examination Required

All students entering 7th grade must have an updated New York State School Health Examination Form on file in the TCMS Health Office.

  • Physical examinations must be submitted on the required New York State form. 

  • The Family Cardiac History section must be completed and reviewed for the physical to be accepted.

Students Entering 8th Grade

There are no additional New York State-required immunizations for students entering 8th grade.

An updated physical examination is not required for 8th grade unless your child:

  • Is new to the district, or
  • Plans to participate in school sports.

Important Deadline

New York State allows only a 14-day grace period after the start of the school year for students to receive required immunizations.

To continue attending school after this grace period, students must:

  • Receive all required immunizations, and
  • Submit documentation to the TCMS Health Office.
